Doc Gumshoe: Will Supplements That Contribute to Health Actually Cure an Illness?

It all starts with an unassailable truth: the healthier you are to start with, the better your odds of fighting off a whole range of illnesses, from Addison’s Disease to Zollinger-Ellison syndrome. The essentials of staying healthy are not deeply mysterious, and if I listed them here, you would likely emit a groan and move on to something more interesting. But another truth, unfortunately also unassailable, is that for most of history, the emphasis in mainstream medicine has been not on keeping people healthy, but on treating sickness. People go to the doctor, not when they want advice on how to stay healthy, but when they are sick. The first medical question the doctor wants an answer to is, “what’s your principal complaint?” If you don’t have a principal complaint, why are you here?

Yes, there have always been wise doctors who stressed staying healthy, and recently health maintenance has been increasingly emphasized by all manner of medical bodies – government, academic, professional organizations, and voluntary groups. Take high blood pressurehypertension – as an example.

The JNC7 Catechism

The Joint National Committee on Prevention, Detection, Evaluation, and treatment of High Blood Pressure (widely known as the JNC) has, in its past several treatment recommendations, emphasized “Life Style Modifications” as the treatment for persons having what they now term “prehypertension.” Here’s how they classified blood pressure in their latest recommendations:

And here’s how they say prehypertension needs to be treated:

“Prehypertension is not a disease category. Rather, it is a designation chosen to identify individuals at high risk of developing hypertension, so that both patients and clinicians are alerted to this risk and encouraged to intervene and prevent or delay the disease from developing. Individuals who are prehypertensive are not candidates for drug therapy based on their level of BP and should be firmly and unambiguously advised to practice lifestyle modification in order to reduce their risk of developing hypertension in the future (see Lifestyle Modifications).”

This is what they say about Lifestyle Modifications:

“Adoption of healthy lifestyles by all persons is critical for the prevention of high BP and is an indispensable part of the management of those with hypertension. … BP is also benefited by an adoption of the Dietary Approaches to Stop Hypertension (DASH) eating plan, which is a diet rich in fruits, vegetables, and lowfat dairy products with a reduced content of dietary cholesterol as well as saturated and total fat. … Dietary sodium should be reduced to no more than 100 mmol per day (2.4 g of sodium). Everyone who is able should engage in regular aerobic physical activity such as brisk walking at least 30 minutes per day most days of the week. Alcohol intake should be limited to no more than 1 oz (30 mL) of ethanol, the equivalent of two drinks per day in most men, and no more than 0.5 oz of ethanol (one drink) per day in women and lighter weight persons. … For overall cardiovascular risk reduction, patients should be strongly counseled to quit smoking.”

What Happens in the So-Called “Real World”

So here’s what happens, at least quite a lot of the time: our patient walks into the doctor’s office, and his blood pressure is 135 over 85. (I made our patient a man, because men are more likely to have high blood pressure than women.) The doctor tells him, without a whole lot of conviction, that he has “prehypertension,” and that he needs to do some lifestyle modification. (I should note here that the concept of “prehypertension” has not had a lot of take-up in the clinical community; most docs have a reasonably good notion of which of their patients are at higher risk for developing established hypertension and which ones can continue in the “prehypertension” range without progressing to the real thing.)

Our patient already knows what’s coming: more exercise, less yummy food, less booze, no smoking. He knows he should follow that plan, and he hopes he’ll have the strength of character to, by gosh, just do it! And maybe, at first, he sort of does follow it. But the problem is, he feels absolutely fine. He has no symptoms, and therefore no regular incentive to stay with the program.

Okay, a year or two later he’s back at the doctor’s, and now his BP is 150 over 95, and the doc tells him that he now definitely does have hypertension. The doc asks him, perhaps more out of duty than really wanting to know, whether he has stuck with his lifestyle modification plan. Maybe he weasels, maybe he fesses up. The doc knows, however, that the chances are that our guy has not stuck with the plan. So now the doc tells our patient that he has got to get on that plan and stick with it, and he’s going to prescribe an antihypertensive drug to go along with it.

Now he’s facing the prospect of being on an antihypertensive drug for the rest of his life. Or maybe two, or even three different antihypertensive drugs.

The Alternative Treatment “Solution”

Then, this pops into his in-box:

Well, we can easily see why our patient would be sorely tempted by these blandishments – live to 120 in good health, why not? And the ingredients in this remedy all sound on the up-and-up – an antioxidant, a source of Omega 3, and Vitamin D3. These couldn’t be bad, and who knows, they might even help with his hypertension.

For the moment, I won’t go into the reservations that have been raised about the effectiveness of antioxidant supplements. I will do no more than suggest that those evil free radicals that they’re supposed to counteract may indeed have some necessary function, and that perhaps we shouldn’t be in a hurry to extirpate then utterly. (There will be more about that in a future commentary.) But here’s the issue:

Will Supplements That Contribute to Health Actually Cure an Illness?

We’re in agreement that the better a person’s baseline health, the better his or her chances are of warding off all manner of illness and disease. But once that disease raises its ugly head, will taking steps that contribute to overall health do much to reverse the course of the disease?

Let’s go back to high blood pressure as an illustrative case. The pathophysiology (medicalese for the processes that lead to the illness) of hypertension is complex, and I’m sure you don’t have the patience for a digest of Braunwald’s 2000 page Heart Disease text. But let me present a simple example of a common cause of high blood pressure, and try to explain how it plays out.

Suppose a person eats a really salty meal. Here’s what happens: first, the concentration of salt in his (yes, it’s a guy again!) blood stream rises. As the blood passes through the kidneys, a precise regulatory process takes place. The sodium concentration in the blood needs to be kept at a constant level, so if it gets too high, the blood needs to be diluted by adding water – i.e., our patient gets very thirsty, drinks water, and the total fluid in his blood stream increases. But the total capacity of his circulatory doesn’t increase, so what happens is that the blood is under greater pressure – i.e., hypertension. Now, this hypertension could be temporary. The kidneys will then pass the excess liquid from the blood into the bladder, and into the toilet it goes, restoring our patient’s BP back to where it was before he scarfed down all those salty snacks.

During that interval, while he was having an episode of hypertension, warning signals were being sent from his circulatory system to his brain. Most large arteries have nerves which sense the pressure within the artery; these are called baroreceptors. The way they work is that when pressure increases in the artery, it stretches to accommodate the pressure, and the nerves stretch, and they send signals to the brain stem saying, “Do Something!” The brain then sends signals that reduce heart rate and relax arterial muscle tone, lowering blood pressure.

These baroreceptors work very well indeed, but for a short time only. When there are rapid changes in blood pressure, the signals go off quickly, at really short intervals. However, their function is short-term modulation of blood pressure. After as short a time as a couple of days, if blood pressure remains elevated, the baroreceptors stop sending these alarm signals, and accept the increased BP as the ”new normal.” This phenomenon is sometimes called “resetting the barostats.”
What that means is that if our patient regularly overloads on salt, his body essentially has no way of sensing that he has high blood pressure. His kidneys will do what they need to do to keep his sodium level at an okay concentration, but as far as blood pressure regulation, his brain now accepts that 150 over 95 is normal. It’s getting no alarm signals from the baroreceptors.

Other Mechanisms of Hypertension

I don’t want to leave you with the impression that every time you eat potato chips, you’re risking a lifetime of drug treatment for high blood pressure. Some people are more salt-sensitive in this regard than others; that seems to be particularly the case in people who are genetically programmed to retain salt, perhaps because their ancestors lived in parts of the world where salt was scarce. Another common mechanism is the renin-angiotensin system (RAS). Renin is an enzyme produced by the adrenal glands, adjacent to the kidneys. Renin eventually becomes a substance called angiotensin, which promotes constriction of the arteries, increasing BP, and boosts the heart’s pumping action, also raising BP. Some individuals are more renin-sensitive than others and therefore more prone to hypertension.

There are many effective and (by and large) quite safe drugs to treat hypertension. I’m not going to try to evaluate the different classes at this point, but if you would like to hear my personal views, please post a comment to that effect and I could devote a piece to those issues.
However, the bottom line here is that once hypertension is established, changing the lifestyle factors that led to hypertension isn’t going to do much to make it go away. Disease processes are in place, and these need to be attacked directly.

But What About Those Healthful Supplements?

Okay, that particular combination consisted of three ingredients, all of which actually do have some healthful credentials.

First, antioxidants. There is some evidence suggesting a connection between oxidative stress and factors that increase blood pressure, and at least one study reported that some hypertensive patients that took a supplement, pycnogenol, based on pine bark, were able to reduce the dosage of their antihypertensive drug by about 30% without their BP rising significantly. But so far data about this association is scant, and antioxidants vary all over the map, so on this front my guess is that treatment for hypertension with antioxidants of any kind is a shot in the dark. And data on the antioxidant in the “Marine D3” preparation, ecklonia cava, is slim.

Second, Omega 3 supplements. These do have a beneficial effect on lipid balance. They tend to raise HDL cholesterol (the good kind), lower triglycerides, and have a small effect on LDL cholesterol (the bad). People with elevated cholesterol frequently also have high blood pressure, but the two conditions need to be treated separately – fixing one doesn’t fix the other, so I don’t see Omega 3’s as an effective high blood pressure remedy.

Third, Vitamin D. Here the story is more interesting. There is strong evidence that persons with low levels of a hormone (calcifediol) which is metabolized in the liver from Vitamin D3, have increased cardiovascular mortality and increased prevalence of hypertension. The suggested mechanism is that Vitamin D3 may inhibit the RAS, through which heart rate and arterial pressure are increased. So you would think that taking supplemental Vitamin D3 might actually lower blood pressure. However, so far, evidence that it actually works as a treatment for hypertension is absent.

My bottom line: these supplements might well help to maintain health, but they’re highly unlikely to control established high blood pressure. Staying healthy and treating disease are two separate projects, and each one has to be addressed individually. This is not only true for hypertension, but for most other diseases, despite the rosy pictures painted by the merchants of alternative treatments.

Interesting comments Doc…
One subject to tackle next time is inflammation and the part it plays in all of this.
More and more research has come out disclosing it to be the # one etiology in
most cardiac and stroke issues.

Thanks, Doc, interesting as always. Those who suffer from poor-health issues are often very vulnerable to the offerings of [perhaps well intended] quacks and greedy health/medical ‘experts.’
Your work undoubtedly saves some folks from disappointments and loss of money.

Inflammation has been found to be required for soft tissue repair after an injury. However, inflammation left go unchecked does cause damage to the circulatory and nervous systems. A discussion on inflammation and how to identify it and how to control
would be very beneficial.

Interesting article,I always enjoy reading about how people prey on other people’s desire to be healthy WITHOUT doing anything different other than taking a “magic pill”.The truth is in order to be healthy everyone needs to moderate what they eat,lower salt intake,exercise at least 4x weekly increasing HR to 50-100% over normal at rest,get a good nights sleep,do not smoke, and limit alcohol consumption.
This is the only “true” way to be healthy and interestingly,most doctors and pharmacologists will tell you that supplements are at best dubious in their effect on the body’s mechanisms and that a good diet trumps anything you can take in pill form.

When I was a medical student many years ago, we employed certain strategies no longer in vogue but which worked quite well. A common medication used back then (I take this for my hypertension) was hydrochlorothiazide. It causes the kidney to waste Sodium but also has the same effect on Potassium. Its effect on Sodium last longer than its effect on Potassium, so we would have the patient not take it on weekends (I don’t). It made my severe hypertension easy to control. I was never a big salt eater but now I use none and use KCl instead. When I need an expectorant , I use KI. Both have keep my Potassium up.

Beta blockers also work well. But very very important is removal of NaCl from the diet. Usually a no added salt diet is all that is needed with very low use of ordinary foods with lots of salt. Works for me.

A pattern of interest regarding all these ‘miracle’ supplements – how come most of them seem to have a base price of $39.99 for a month’s supply?
This seems to be what these marketers have discovered ‘the traffic will bear’.
And some of the most popular ingredients, such as Vit D3, cost literally pennies.
They often quote the aggregate price of taking theiingredients separately, but don’t ever seem to be looking for the best prices!

My late mother who died of a stroke and as a widow became was part of a trial of hydrochlorothiazide run by (Dr. Ralph; full name available if anyone needs it at Columbia-Presbyterian Hospital in New York). Mom was told to eat a banana every day to offset the drug which not only cut her sodium, but also her potassium. The banana replaced the potassium, apparently. I am not sure what her drug regimen was being trialed against or how she was designated for the trial. She had a mini-stroke at age 80 or 81 (I cannot recall exactly when) and recovered but then was hit by a big one in 1997 and after about 18 months of severe disability and frustration died in 1999 aged 89. I am ultimately unsure if the hydrochlorothiazide helped but presumably the trial records would show this.

Hmmm…. your article is on target but your title seems off base…… alternative therapies are wide ranging (acupuncture, holistic, homeopathy) yet your main theme is supplements. While I agree that the marketing of magic bullets is rampant, that applies to western medicine as well as the vitament supplement industry . Your idea of the responsibility of the patient and his or her lifestyle is key. Yet we are bombarded in the media with the idea that the little purple pill (as example) can resolve the problem indicated and while encouraged to change lifestyle in the small print, the overall message is have your doctor write a prescription. Alternative therapies are meant to encourage the body to heal itself rather than target a specific “disease”. Patient responsibility towards reaching that goal is key no matter what modality you use. The first rule of medicine is to “do know harm” and I think that message has been overlooked by the pharmaceutical companies in search of huge dollars. Case in point is that there are more prescription drug addicts than any other form of drug addiction. The physiology of the human body is truly a marvel as it works every second to keep us upright and functioning. To support that physiology should be the goal, both by patient and practitioner.

what I was trying to say was that all the ethical lapses were not only by the vitamin peddlers with their $39.95. My mother was put on hydrochlorothizide for a trial without either her nor
her daughter being briefed about the risks. Moreover Dr. Ralph did not meet the gold standard for trials because she and he both knew she was on hydrochlorothizide and not getting a statin. In a proper trial neither the doctor nor the patient knows which arm the patient is in. What I wonder is whether the counterpart 80-odd year old thin little old lady who got the statin did better than mom in avoiding a massive stroke or not. I suppose I will have to try to find the final report the medic wrote on the trial.

here is the only article I could find. Mom lived in Northern Manhattan and was among the c18% of patients there who were counted as European; she was born in Germany.

Whether the treatment of patients with hypertension who are 80 years of age or older is beneficial is unclear. It has been suggested that antihypertensive therapy may reduce the risk of stroke, despite possibly increasing the risk of death.

We randomly assigned 3845 patients from Europe, China, Australasia, and Tunisia who were 80 years of age or older and had a sustained systolic blood pressure of 160 mm Hg or more to receive either the diuretic indapamide (sustained release, 1.5 mg) or matching placebo. The angiotensin-converting–enzyme inhibitor perindopril (2 or 4 mg), or matching placebo, was added if necessary to achieve the target blood pressure of 150/80 mm Hg. The primary end point was fatal or nonfatal stroke.

Results

The active-treatment group (1933 patients) and the placebo group (1912 patients) were well matched (mean age, 83.6 years; mean blood pressure while sitting, 173.0/90.8 mm Hg); 11.8% had a history of cardiovascular disease. Median follow-up was 1.8 years. At 2 years, the mean blood pressure while sitting was 15.0/6.1 mm Hg lower in the active-treatment group than in the placebo group. In an intention-to-treat analysis, active treatment was associated with a 30% reduction in the rate of fatal or nonfatal stroke (95% confidence interval [CI], −1 to 51; P=0.06), a 39% reduction in the rate of death from stroke (95% CI, 1 to 62; P=0.05), a 21% reduction in the rate of death from any cause (95% CI, 4 to 35; P=0.02), a 23% reduction in the rate of death from cardiovascular causes (95% CI, −1 to 40; P=0.06), and a 64% reduction in the rate of heart failure (95% CI, 42 to 78; P<0.001). Fewer serious adverse events were reported in the active-treatment group (358, vs. 448 in the placebo group; P=0.001).

If you can, please publish a discussion of vitamin D/D3?
I am well past ‘full’ retirement’ age. Had several severe, common, medical events in the standard age range times. Came to a stage where more things inside me seemed to be in a state of decline and was severely limited in what foods/liquids I could eat. I found something in one of those ads for a ‘miracle’ fix (probably on a car radio ‘infomercial’ ??) just like the patter you described at the beginning of your discussion today; that made me add (the cheapest version I could find which was a store-brand) vitamin D to my daily intake of pills. My overall apparent health improved dramatically over a few months, I can eat almost anything I want again (nearly a year after starting the Vitamin D3 supplement). I have no way of knowing if the vitamin D3 was co-incidental or if it was at least partly causative. My doctors will not comment or give any clues in any way.

To muddy the water, at the same time I began ‘Vinegar Therapy’ again. Vinegar Therapy is quite popular (for those who have never heard about it) around the world. Apparently Citric Acid in small amounts is good for us humans. Like the juice of a fresh lemon—- every day. The acid(s?) in vinegar also work to some extent. In America the vinegar of choice is Apple Cider Vinegar. In Japan it is specific Rice Wine Vinegars. The procedure is to take a tablespoon or so of vinegar by mouth two or three times a day. Wash it down with water. Keep the vinegar off your teeth: the acids attack tooth enamel and in this ongoing approach the cumulative effect will be highly undesirable for your teeth. Research it before trying it! I take a shot glass brim full of vinegar once a day. Straight– it will knock you back like a shot of rotgut whiskey!! Good for sore throats if you live through the choking burn! (ROTFL!!!). Best if mixed with a cup or more of water and then follow with more water. Mostly I use a lot of water and my tummy aches for a half-hour after. Some food (lunch?) soon after helps considerably.

For other readers: Research Vitamin D on the internet, at least, before messing with it. All Vitamin D is D3 by the time it enters the fluids of the body. The manufacturing process is straightforward, the end result is a standard item/chemistry. Paying more than the minimum possible is silly. Vitamin D/D3 is fat soluble and can build up in the body and become a problem. I take one tiny liquid-filled capsule every-other day and sometimes miss for a few days on purpose.

It would be very interesting to see what you might have to say about either Vitamin D/D3 or the ‘Vinegar Therapy,’ or both. Or tell how to find older articles if you already have done these?

Well this is disappointing. Yet another shill for the cut-burn-and-poison school of treat-the-symptoms-maximize-the-expenses style of medical greed-mongering. The author states in the disclaimer that he’s not an MD, yet takes the exact stance approved by the AMA (which is a high-pressure, special-interest political lobbying group).

The author assures us that, beyond a number established by the AMA, a person with elevated blood pressure CANNOT heal his ir her own body. Above 150? Too bad, sucker! NOW you MUST take our poisonous harmaceuticals for the rest of your LIFE. Your body has not ability to heal itself, no matter WHAT you do! So, pay up and begin the process of destroying your body slowly while being assured that your lowered blood pressure is a “cure” to the “illness” We the MDs have defined.

The truth is quite different. In fact, every part of the body (yes, even the teeth and eyes) CAN heal itself, given a correction of the environmental stimuli which have forced it into a diseased state. The AMA does not want people to know about this, because they make a fine living off of our suffering. AMA MDs do not try to “cure” any more. Instead, they prefer to “treat” the symptoms and suppress the body’s natural healing mechanisms. Any MD who speaks out against this harmful tyranny is persecuted by the politically powerful AMA. Anyone who speaks of “cure” becomes an instant target for political, economic and sometimes even physical assassination.

This writer subscribes to newsletters from half a dozen practicing MDs who espouse medical freedom and encourage the body’s natural healing mechanisms. The AMA persecutes these heretics using every avenue available to them. Don’t be misled. Don’t let an AMA cut-burn-and-poison quack rob you of your health and quality of life. It’s YOUR health; anyone who is content to prescribe you a harmaceutical for life is NOT even trying to cure your illness or disease. Look elsewhere; there are more doctors every day leaving the AMA’s tyrannical embrace in favor of doing no harm, and actually healing people. Help in healing is out there; thank the AMA for the difficulty you’ll have in finding qualified practitioners.

Immediately below the disclaimer in today’s article (“Is There a Common Theme in Alternative Medical Therapies?”) there is a cartoon (an ad, in truth) about “1 Tip for a tiny belly”.

The link takes the reader to a spiel about losing weight with Garcinia Cambogia.

Was the placement of this ad an example of Murphy’s Law, or does the medo-thinkolator have a sense of humor?

Ha! Ads are served up automagically by Google, so everyone sees different ads, but we do sometimes see ads from the very newsletters we’re covering crop up next to the articles.

Studies have shown that only about half the population is susceptible to blood pressure increase due to salt intake. Furthermore, the real issue is how much potassium a person ingests relative to sodium, which has a lowering effect on BP. Additionally, natural, unrefined sea salt contains many minerals which seem to render the salt quite innocuous in its influence on BP, unlike the high amount of white, refined table salt Americans consume. I know of an Alternative health physician (granted he is a vegetarian) who says he regularly consumes high amounts of Himalayan (natural unrefined) salt and his Blood pressure is less than 100/70. There are hundreds of complimentary physicians around the country who are MD’s and who successfully treat elevated BP through diet, targeted supplements and exercise. Read up on celery juice (high in potassium) and beet juice (high in Nitric Oxide) as effective dietary solutions. While there is plenty of hype in the advertising world and purveyors of natural food products and vitamins are not immune to such excesses (in all things caveat emptor), hypertension and many other ailments, both chronic and mild can be treated and cured using natural means.Check with your (integrative, complimentary, holistic, naturopathic) physician before trying any approach.

About vitamin D3: it’s better if you get it from sunlight during the spring, summer, and fall. Depending on how naturally dark your skin is, you need 15-60 minutes of daily noon-day sun (with no sunscreen).

One investigator found regular sunlight exposure produces the following:
1) Vitamin D3 (fat soluable)
2) Vitamin D3 sulfate (water soluable)
3) cholesterol sulfate (water soluable)
4) Nitric Oxide (lowers blood pressure by expanding the arteries).
5) 5th yet undetermined substance that affects health positively.

There may be more. Vitamin D3 supplimentation is only recommended by some doctors during the winter and sunlight during the spring, summer, and fall is recommended instead.

During the winter, tanning beds are recommended by some doctors instead of Vitamin D3 supplimentation, however the right kind of tanning bed must be chosen to avoid skin cancer.

Thank you. This is more definitive than most of what I was able to find. 15 to 60 minutes in the sun every day? I have never seen a timeframe for length of sun exposure of any kind before. Also, the additional things you listed are new to me. Not things that advance the agendas of most sources of information. I will struggle with Google limiting the search engine results to what they have paid ads to promote and see what I can find.
